Key points

  • Falling oestrogen moves fat to the abdomen, even without eating more.
  • Protein, strength training and sleep matter more than ever.
  • GLP-1 medicines work after menopause; your doctor checks heart and bone health first.
  • Early evidence suggests hormone therapy may add to GLP-1 weight loss.

Why weight changes around menopause

In the years around menopause, usually from the mid-40s to the early 50s for Indian women, many women gain weight or find their shape changes even when they eat the same. Several things happen at once:

  • Falling oestrogen moves fat from the hips and thighs to the abdomen.
  • Muscle declines with age, so the body burns a little less energy.
  • Hot flushes and night sweats disrupt sleep, which drives hunger and cravings.
  • Life changes, stress and less activity often add up.

Fat around the abdomen matters most, because it raises the risk of diabetes, heart disease and fatty liver. For Indian women, a waist of 80 cm or more is above the risk level.

What helps

  • Protein at every meal and strength training two to three times a week, to protect muscle and bone.
  • Fewer refined carbs and sugar, as insulin resistance often rises at this stage.
  • Better sleep: treat hot flushes, and keep a regular bedtime.
  • Limiting alcohol, which adds calories and worsens hot flushes and sleep.
  • Calcium and vitamin D for bone health, as your doctor advises.

Can GLP-1 medicines help after menopause?

Yes. Mounjaro and semaglutide work in women after menopause, and the main trials included many women in this age group. Your doctor will look at your heart, bones and any other medicines before prescribing.

There's early evidence that menopause hormone therapy may add to the effect: in a 2026 Mayo Clinic study of women on tirzepatide, those also using hormone therapy lost about 35% more weight than those who weren't. The study was observational, so it can't prove cause and effect, and hormone therapy is only for women whose gynaecologist or doctor recommends it for menopause symptoms.

Protect your muscle and bones

Weight loss after 50 can mean losing muscle and bone as well as fat. That's why we track body composition with InBody scans and build protein and strength into every plan. See protecting muscle on GLP-1 treatment and our 7-day exercise plan.

When to see a doctor

  • Your waist is 80 cm or more, or you've gained a lot of weight quickly.
  • Your periods have become very heavy or irregular, or you bleed after menopause.
  • Hot flushes, sleep or mood are affecting your daily life.
  • You have diabetes, high blood pressure or fatty liver.

Questions

Why do I gain weight around menopause?

Falling oestrogen, loss of muscle with age, poor sleep from hot flushes and life changes all add up, and fat tends to move to the abdomen.

Is it harder to lose weight after menopause?

It can be, but it's very possible. Protein, strength training, sleep and, for some women, medical treatment all help.

Can I take Mounjaro or Wegovy after menopause?

Many women can, if a doctor agrees after an assessment. They work in this age group.

Does HRT cause weight gain?

Research doesn't show that hormone therapy itself causes weight gain. Talk to your gynaecologist about whether it's right for you.
