Key points

  • About 1 in 10 urban Indian adults has hypothyroidism; a third don't know it.
  • It usually causes only a few kilos of weight gain.
  • Most people with treated hypothyroidism can take GLP-1 medicines.
  • GLP-1s are not used with a personal or family history of medullary thyroid cancer or MEN2.

How your thyroid affects weight

Your thyroid gland makes hormones that set how fast your body uses energy. When it's underactive (hypothyroidism), you may gain a few kilos, feel tired and cold, and find it harder to lose weight. When it's overactive (hyperthyroidism), you may lose weight without trying.

Hypothyroidism is common in India: a study in eight Indian cities found it in about 1 in 10 adults, and about a third of them didn't know they had it. It's much more common in women.

How much weight does hypothyroidism cause?

Usually less than people think: often a few kilos, part of it fluid. If you've gained a lot of weight, hypothyroidism is rarely the only cause. Once your thyroid is treated, that thyroid-related weight often settles, but the rest needs the usual approach: food, activity, sleep, and sometimes medicine.

Getting your thyroid checked

A simple blood test for TSH, sometimes with free T4, shows whether your thyroid is underactive. Lean Life doctors check it as part of your first assessment if it hasn't been done recently.

Can you take Mounjaro or semaglutide with hypothyroidism?

Yes, for most people. Treated hypothyroidism doesn't stop you taking GLP-1 medicines. Your doctor will check that your thyroid levels are stable first.

Important: GLP-1 medicines are not used if you or a close family member has had medullary thyroid cancer, or if you have MEN2, a rare inherited condition. Tell your doctor about any thyroid lumps or thyroid cancer in your family.

Taking thyroid tablets correctly

  • Take levothyroxine on an empty stomach with water, 30 to 60 minutes before breakfast or tea.
  • Keep calcium, iron and antacid tablets at least 4 hours away from it.
  • Take it at the same time every day.
  • Get your TSH rechecked about 6 to 8 weeks after any dose change, and after you lose a lot of weight, as your dose may need adjusting.

A thyroid-friendly diet for weight loss

  • Use iodised salt, but don't overdo salt.
  • Cooked cabbage, cauliflower and broccoli are fine; there's no need to avoid them.
  • Eat soy at a consistent time, away from your thyroid tablet.
  • Build meals around protein and vegetables, as for any weight-loss plan.
  • Skip thyroid 'detox' teas and supplements; they don't work and some contain hidden thyroid hormone.

Questions

Can hypothyroidism stop you losing weight?

Untreated, it can make it harder. Once your thyroid levels are normal, you can lose weight like anyone else.

Will thyroid tablets make me lose weight?

They correct the thyroid-related part, often a few kilos. They're not a weight-loss medicine.

Is Mounjaro safe if I have thyroid problems?

For treated hypothyroidism, usually yes. It's not used if you or a close relative has had medullary thyroid cancer, or if you have MEN2.

What is a good diet for thyroid weight loss?

The same as any healthy weight-loss diet: protein and vegetables at every meal, fewer refined carbs and sugar, and iodised salt.
