Key points
- Surgery usually brings more weight loss and lasts without ongoing medicine.
- GLP-1 medicines avoid an operation but usually need to be continued.
- Surgery costs about ₹3 to 5.5 lakh once; medicines cost ₹1,500 to ₹25,781 a month at MRP.
- For Asians, guidelines say surgery should be offered from BMI 27.5.
The short answer
Bariatric surgery usually leads to more weight loss than medicines, and the effect lasts without ongoing treatment, but it's an operation with recovery time and lifelong changes to how you eat. GLP-1 medicines like Mounjaro and semaglutide avoid surgery and can be stopped or adjusted, but most people need to stay on them, or on a lower dose, to keep the weight off. Many people can now consider medicines first.
Side by side
| Bariatric surgery | GLP-1 medicines (Mounjaro, semaglutide) | |
|---|---|---|
| What it is | An operation, usually sleeve gastrectomy or gastric bypass, that makes the stomach smaller and changes gut hormones | A weekly injection that copies gut hormones, reducing appetite and slowing the stomach |
| Typical weight loss | Often around a quarter to a third of body weight | About 15% on average with semaglutide 2.4 mg and up to about 21% with tirzepatide 15 mg in the main trials |
| Who it's for | Usually BMI 35 or more, or lower with diabetes or other conditions; for Asians, guidelines say surgery should be offered from BMI 27.5 | BMI 30 or more, or 27 or more with a weight-related condition |
| Cost in India | About ₹3 to 5.5 lakh, one time, more in big-city hospitals | Medicine MRP from about ₹1,500 to ₹25,781 a month, ongoing |
| Insurance | Often covered if you meet medical criteria | Usually not covered |
| Recovery | Hospital stay of a few days and a few weeks' recovery | No downtime; side effects like nausea usually ease |
| Lifelong needs | Vitamin and mineral supplements and regular check-ups | Usually continued treatment to keep the weight off |
| Can it be undone? | Sleeve gastrectomy can't; bypass rarely is | Yes, the dose can be changed or stopped |

When surgery may be the better choice
- Very high BMI, for example 40 or more, where medicines alone may not get you to a healthy weight.
- Type 2 diabetes that stays poorly controlled despite treatment.
- You've tried medicines and they didn't work well enough, or you couldn't tolerate them.
- You'd rather have a one-time treatment than take medicine long term.
When medicines may be the better choice
- A BMI between 27 and 35, especially without severe complications.
- You want to avoid an operation, or surgery is risky for you.
- You want something that can be adjusted or stopped.
- As a step before surgery: some surgeons use medicines to lower weight and risk first.
Insurance in India
Many Indian health insurance policies cover bariatric surgery when it's medically necessary, typically for adults with a BMI of 40 or more, or 35 or more with conditions such as diabetes, heart disease or severe sleep apnoea. Waiting periods and sub-limits often apply, so check your policy wording. GLP-1 medicines taken at home are usually not covered.
How Lean Life helps you decide
Lean Life doesn't perform surgery. Our doctors look at your weight, health and goals and explain both routes honestly. If medicines are a good fit, we can start you on a programme; if surgery looks like the better option, we'll say so and point you to a bariatric surgeon.

Can I take GLP-1 medicines after bariatric surgery?
