What Mounjaro is

Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly. It launched in India in March 2025 and is licensed for type 2 diabetes and for chronic weight management. There is no generic tirzepatide in India.

How it works

Tirzepatide acts on two gut hormones, GIP and GLP-1, which normally rise after a meal. It slows how quickly the stomach empties, reduces hunger and helps the body manage blood sugar. In the SURMOUNT-1 trial, adults on the highest dose (15 mg) lost 20.9% of their body weight on average over 72 weeks, alongside diet and activity changes.

What the trials showed

TrialWho took partAverage weight loss
SURMOUNT-1 (72 weeks)Adults with obesity, without diabetes15.0% at 5 mg, 19.5% at 10 mg, 20.9% at 15 mg (placebo 3.1%)
SURMOUNT-2 (72 weeks)Adults with obesity and type 2 diabetes12.8% at 10 mg, 14.7% at 15 mg (placebo 3.2%)
SURMOUNT-3 (72 weeks)After a 12-week intensive diet programmeA further 18.4% on tirzepatide, while people on placebo regained 2.5%
SURMOUNT-4 (88 weeks)People who had already lost weight on tirzepatideThose who stayed on it lost a further 5.5%; those switched to placebo regained 14%
SURMOUNT-5 (72 weeks)Tirzepatide vs semaglutide, head to head20.2% on tirzepatide vs 13.7% on semaglutide

All trials combined medicine with a reduced-calorie diet and physical activity. Individual results vary.

Two things stand out for Indian patients. People with type 2 diabetes usually lose a little less than people without it. And stopping treatment usually brings much of the weight back, so it helps to plan from the start how you'll keep it off.

Who it's for

Mounjaro is licensed for adults with a BMI of 30 or more, or 27 or more with a weight-related condition such as prediabetes, type 2 diabetes, high blood pressure or sleep apnoea. Indians tend to develop these conditions at lower weights, which is why Indian guidelines define obesity from a BMI of 25. Your doctor weighs both.

It is not for anyone who is pregnant or breastfeeding, or who has a personal or family history of medullary thyroid cancer or MEN2. People with past pancreatitis need careful assessment. Read the full safety guide.

Using the contraceptive pill? Tirzepatide can make the contraceptive pill less reliable. If you use the pill, switch to a non-pill method or add a barrier method such as condoms for 4 weeks after starting and for 4 weeks after each dose increase.

Vials are sold in India at 2.5 mg and 5 mg only, where four vials cost about the same as one KwikPen. Higher strengths come only as the KwikPen. Health insurance in India usually doesn't cover these medicines for weight loss, so check your policy. Work out your first-year cost.

Mounjaro usually brings more weight loss; semaglutide, especially as a generic, costs far less. Your doctor helps you weigh results, side effects and budget. Full comparison.

Dosing

Treatment starts at 2.5 mg once a week for four weeks, then 5 mg. Your doctor may increase it in 2.5 mg steps, at least four weeks apart, to a maintenance dose of 5, 10 or 15 mg. If you miss a dose, take it within 4 days; otherwise skip it. See the full dosing guide or check a missed dose.

Side effects

The most common side effects are nausea, diarrhoea, constipation and vomiting, mostly while the dose is going up. In SURMOUNT-1, about 3 in 10 people on the higher doses had nausea, compared with 1 in 10 on placebo. Rarer but serious risks include pancreatitis and gallbladder problems. Read the side effects guide. You can report side effects to your doctor and to the Pharmacovigilance Programme of India (PvPI).

Storing Mounjaro in Indian heat

  • Before first use, keep the KwikPen in the fridge at 2 to 8°C, away from light. Don't freeze it.
  • Once you start a pen, you can keep it at room temperature up to 30°C for up to 30 days. Throw it away after 30 days or four doses, whichever comes first.
  • Indian rooms often go above 30°C in summer, so keep pens in the fridge unless you're travelling.
  • Travel with an insulated pouch and cool pack, in your hand luggage on flights. Never leave pens in a car or in sunlight.

Checking it's genuine

The World Health Organization has warned about fake and substandard weight-loss injections. Protect yourself:

  • Buy only with a prescription, from a licensed pharmacy. Never from social media sellers, gyms or salons.
  • Check the pack is sealed, the batch number and expiry date are printed clearly, and the price is not far below MRP.
  • Pens and vials should arrive cold, in insulated packaging.
  • If anything looks wrong, don't inject it. Show it to your pharmacist or doctor.

Who tends to do best on Mounjaro

  • People who reach and stay on an effective dose, rather than stopping early.
  • Those who eat enough protein and do strength training, which protects muscle.
  • People without type 2 diabetes, who on average lose a little more.
  • Those who plan for the long term, including how to step down.

Eating well on Mounjaro

Your appetite drops, so every meal counts. Start with protein (dal, paneer, curd, eggs, chicken or fish), then vegetables, then a small portion of roti or rice. Eat slowly, stop when comfortably full, and go easy on oily and very spicy food, which can make nausea worse. See our vegetarian and non-vegetarian 7-day plans and the protein calculator.

Common myths

  • 'It's only for diabetes.' In India it's licensed for weight management too.
  • 'There's a Mounjaro tablet.' There isn't; it's a weekly injection.
  • 'There's a cheaper generic Mounjaro.' Not in India; only semaglutide has generics.
  • 'You can stop once you hit your goal.' Most people regain weight after stopping, so plan the step-down with your doctor.

Questions

Is Mounjaro the same as Ozempic?

No. Mounjaro is tirzepatide, which acts on two hormones (GIP and GLP-1). Ozempic is semaglutide, which acts on one (GLP-1), and is licensed in India for type 2 diabetes. See the full comparison.

Can I buy Mounjaro without a prescription?

No. It is a prescription-only medicine. Buying it without one is unsafe and risks getting a fake product.

How long do I need to take it?

Weight management with tirzepatide is usually long-term. In the SURMOUNT-4 trial, people who stopped regained much of the weight they had lost. Your doctor plans how long to continue and how to step down.

Can I use Mounjaro if I'm vegetarian?

Yes. The medicine doesn't depend on diet type, but getting enough protein matters. Our dietitians build vegetarian and Jain plans.

Is the vial or the KwikPen better?

Both contain the same medicine. Vials are only sold at 2.5 mg and 5 mg, where they cost about the same per month as the pen. The pen is simpler to use; vials need a syringe and careful measuring.
