Key points
- Losing weight often improves blood sugar control in type 2 diabetes.
- Tirzepatide and semaglutide are approved for type 2 diabetes in India.
- Average weight loss in people with diabetes: about 13% to 15% on tirzepatide, about 10% on semaglutide 2.4 mg.
- Insulin and sulfonylurea doses may need lowering to avoid low blood sugar.
How weight and type 2 diabetes are linked
Extra weight, especially around the waist, makes the body less responsive to insulin, so blood sugar rises. For many people with type 2 diabetes, losing weight improves blood sugar control, and some can reduce their diabetes medicines with their doctor's guidance.
GLP-1 medicines for people with diabetes
GLP-1 medicines were first developed for diabetes. In India, tirzepatide (Mounjaro) and semaglutide (Ozempic, Rybelsus and several generics) are approved for type 2 diabetes, and tirzepatide and some semaglutide brands are also approved for weight management.
People with diabetes usually lose a little less weight on these medicines than people without. In trials of adults with type 2 diabetes and overweight, tirzepatide led to an average loss of about 13% to 15% of body weight over 72 weeks (SURMOUNT-2), and semaglutide 2.4 mg about 10% over 68 weeks (STEP 2).
What your doctor checks
- Your HbA1c, fasting sugar, kidney function and eyes.
- Every diabetes medicine you take. Insulin and sulfonylureas (such as glimepiride or gliclazide) may need lower doses to avoid low blood sugar.
- Any diabetic eye disease. Fast improvements in blood sugar can, rarely, worsen it, so eye checks matter.
- Your diet and activity, so the plan fits your day.
How Lean Life works with your diabetes care
We work alongside your regular doctor or diabetologist. Our endocrinologist, Dr Johann Varghese, sees patients whose weight is linked to diabetes, thyroid or other hormonal conditions. We track HbA1c and InBody results and share them with your doctor if you'd like.
Questions
Can GLP-1 medicines help diabetes and weight together?
Yes. They lower blood sugar and reduce appetite, and are approved for type 2 diabetes in India. Some are also approved for weight management.
Will I lose as much weight as someone without diabetes?
Usually a little less. Trials in people with diabetes showed about 13% to 15% average loss on tirzepatide and about 10% on semaglutide 2.4 mg.
Can I stop my diabetes medicines?
Not on your own. Your doctor may reduce some as your sugar improves.
Do I need to see a diabetologist?
Your existing doctor stays involved. Our endocrinologist can also see you at Lean Life.
