Key points
- The first month is mainly about adjusting to the medicine.
- Steady loss usually builds over months 2 to 6.
- Average loss levelled off at around 60 to 72 weeks in the trials.
- Muscle matters: we track fat and muscle with InBody.
A typical timeline
| When | What usually happens |
|---|---|
| Weeks 1 to 4 (starting dose) | Appetite starts to drop and portions shrink. Weight may begin to fall, but this dose is mainly to let your body adjust. Side effects like nausea are most likely now. |
| Months 2 to 3 | Doses step up and weight loss becomes steady. Many people notice clothes fitting differently and more energy. |
| Months 4 to 6 | Often the period of fastest progress for many people. Your doctor may hold your dose if you're losing well. |
| Months 6 to 12 | Weight loss continues but slows. Plateaus are common and normal; your doctor and dietitian adjust the plan. |
| After about a year | In the main trials, average weight loss levelled off at around 60 to 72 weeks. The focus moves to keeping the weight off. |
A general pattern from trials and clinical experience. Everyone's journey is different.
What the trials found
- Tirzepatide (SURMOUNT-1): average loss of 15%, 19.5% and 20.9% of body weight at 5 mg, 10 mg and 15 mg after 72 weeks.
- Semaglutide 2.4 mg (STEP 1): average loss of about 15% after 68 weeks.
- In both, results came alongside a reduced-calorie diet and more physical activity.
What affects your results
- Your starting weight and health, including diabetes, which usually means slightly less weight loss.
- The dose you reach and stay on.
- Protein intake and strength training, which protect muscle.
- Sleep, stress and alcohol.
- Sticking with the plan through plateaus.
Why we measure more than weight
The scale can't tell fat from muscle. Our InBody scans show how much fat you've lost and how much muscle you've kept, so your plan protects your strength. One of our patients, Mehernosh Captain, lost 25 kg in four months while keeping his muscle mass. See patient results.
Questions
How fast will I lose weight on Mounjaro?
Most people notice appetite changes in the first weeks, with steady loss building from the second or third month. Your doctor sets a realistic target for you.
Why am I not losing weight in the first month?
The first month is a low starting dose to help your body adjust. Weight loss usually picks up as the dose increases.
Is it normal to hit a plateau?
Yes. Plateaus are common. See our plateau guide.
