Key points
- Ozempic 1 mg led to about 6.5 kg weight loss in people with diabetes.
- Wegovy 2.4 mg, the same medicine at a higher dose, led to about 15% loss.
- Most loss happens in the first 9 to 12 months, then levels off.
- Photos don't show whether you lost fat or muscle.
What results really look like
Ozempic and Wegovy both contain semaglutide. Most dramatic before-and-after photos online come from people on Wegovy's higher weight-loss dose, or on tirzepatide (Mounjaro), and many are edited or unverified. Here's what trials actually show.
| Medicine and dose | Who was studied | Average weight loss |
|---|---|---|
| Ozempic, semaglutide 1 mg (SUSTAIN 7) | Adults with type 2 diabetes, 40 weeks | About 6.5 kg |
| Wegovy, semaglutide 2.4 mg (STEP 1) | Adults with obesity, without diabetes, 68 weeks | About 15% of body weight |
| Wegovy, semaglutide 2.4 mg (STEP 2) | Adults with obesity and type 2 diabetes, 68 weeks | About 9.6% of body weight |
| Mounjaro, tirzepatide 15 mg (SURMOUNT-1) | Adults with obesity, without diabetes, 72 weeks | About 21% of body weight |

A typical timeline
- Month 1: appetite drops; weight loss is usually small while the dose is low.
- Months 2 to 4: steady loss as the dose rises; clothes start to fit differently.
- Months 4 to 9: often the fastest progress, then a gradual slowdown.
- After about a year: weight usually levels off, and the focus moves to keeping it off.
More detail: results month by month.
Why photos can mislead
- Lighting, posture, clothes and editing can change a photo more than a few kilos.
- Celebrity transformations often involve trainers, chefs and other treatments.
- Photos don't show whether the weight lost was fat or muscle.
- They also don't show what happened after the photo, including weight regain after stopping.
What about 'Ozempic face'?
Fast weight loss can make the face look thinner or older. It isn't caused by the medicine itself, and losing weight more steadily, eating enough protein and staying hydrated all help. Read about Ozempic face.

How we track real results
At Lean Life, every patient has regular InBody scans, which show how much fat and muscle you've lost, not just your weight. One patient, Mehernosh Captain, lost 25 kg in four months while keeping his muscle mass. See patient results.
