Fatty liver and weight loss: what helps, and what GLP-1 medicines have shown

Fatty liver is very common in India and closely tied to weight. Here's why weight loss is the main treatment, and what the trials of semaglutide and tirzepatide have shown.

Medically reviewed and approved by Dr Reina Punj, MBBS, PG Diploma in Endocrinology (University of South Wales, UK), Lead Physician, Lean Life. Approved on 24 October 2026.

Key points

  • Fatty liver (MASLD) is closely linked to weight, insulin resistance and diabetes.
  • Steady weight loss is the main treatment.
  • In trials, semaglutide and tirzepatide helped resolve liver inflammation in many people with MASH.
  • In India they're approved for weight and diabetes, not for fatty liver itself.

What fatty liver is

Fatty liver, now called MASLD (metabolic dysfunction-associated steatotic liver disease), means fat building up in the liver. It is very common in India and closely linked to extra weight, insulin resistance and diabetes. In some people it becomes MASH, where the fat causes inflammation that can scar the liver (fibrosis) over time.

Why weight loss matters

Losing weight is the main treatment for fatty liver. Steady weight loss lowers liver fat, and larger, sustained weight loss can calm inflammation and, in some people, improve scarring. Crash diets aren't the answer: gradual loss with good nutrition is safer for the liver.

What GLP-1 medicines have shown

  • Semaglutide 2.4 mg (ESSENCE trial). In adults with MASH and moderate to advanced scarring, 63% on semaglutide had their liver inflammation resolve without worse scarring after 72 weeks, compared with 34% on placebo. In August 2025, the US FDA approved Wegovy for this condition.
  • Tirzepatide (SYNERGY-NASH trial). In a phase 2 study of people with MASH and scarring, liver inflammation resolved in 44% to 62% of those on tirzepatide, depending on dose, compared with 10% on placebo, at 52 weeks.

In India, these medicines are approved for weight management and diabetes, not for fatty liver itself. If you have fatty liver and meet the criteria for weight-management treatment, your doctor may consider them, alongside diet and activity.

What your doctor checks

  • Liver blood tests and, where needed, a scan such as an ultrasound or FibroScan to look at fat and stiffness.
  • Blood sugar, cholesterol and blood pressure, which often travel with fatty liver.
  • Alcohol intake, which matters a great deal for liver health.
  • Any medicines that can affect the liver.

Questions

Can fatty liver be reversed?

Often, yes, especially in its early stages, with steady weight loss, good nutrition and less alcohol.

Is Wegovy approved for fatty liver in India?

Not specifically. In the US it was approved in 2025 for MASH with moderate to advanced scarring. In India it's approved for weight management.

How much weight do I need to lose?

Your doctor will set a target. Even modest loss lowers liver fat; larger sustained loss helps more.

Do I need a FibroScan?

Sometimes. Your doctor decides based on your blood tests and other risks.
